LifeStream Services is accepting nominations for Awards of Excellence to honor those who have gone the extra mile to help seniors, people with disabilities and their communities in East Central Indiana.
Nominations can be made in any of the following categories:
Chairman’s Award: To honor an individual of any age, organization, association or government leader who has done the most to positively impact the lives of seniors or people with disabilities who want to remain independent.
Champion Award: To honor an individual of any age who has successfully advocated on behalf of seniors or people with disabilities.
Focus Award: To honor an organization, individual of any age or team who has put the spotlight on issues affecting seniors or people with disabilities, either through media attention or other means.
Golden Hoosier Award: To honor an individual age 60 or older in Grant, Jay, Blackford, Randolph, Henry, Delaware or Madison counties who has given a lifetime of service and improved the quality of life in his or her community. This award is in conjunction with a state award program coordinated by the Division of Aging and the Lieutenant Governor’s Office.
Nominations should include the name, address and contact information for the recipient and nominator along with information why the person is being nominated.

Dean’s list
Jason Pottkotter, Fort Recovery, was named to the dean’s list at the University of Northwestern Ohio, Lima, for the winter quarter. John Link and Derek Wendel, of Fort Recovery, were named to the dean’s list at the College of Applied Technologies at Northwestern.
Shelby Brunswick, Greg Kahlig and Kayla Pottkotter, all of Fort Recovery, have been named to the dean’s list at the University of Findlay (Ohio) for the fall semester.[[In-content Ad]]
