Cook, 53, Dunkirk, operating a vehicle while intoxicated, a Class C misdemeanor — Sentenced to 60 days in Jay County Jail with all but 22 days suspended, given two days credit for one day served, assessed court costs of $183.50, ordered to pay an alcohol and drug countermeasures fee of $200 and a substance abuse program fee of $400 and placed on formal probation for 11 months. As part of a plea agreement, a charge of operating a vehicle with an alcohol concentration equivalent of 0.08 percent or more was dismissed.
