TO: THE CHILDREN OF HURLESS L. SCHWARTZKOPF AND/OR THE HEIRS AT LAW OF FRANK J. SCHWARTZKOPF AND DOROTHY J. KEMP

To the above person named as Respondent and any other person who may be concerned.

You are notified that you have been named as a beneficiary in the above-named document which has been docketed in the court above named. Mainsource Bank is the Sucessor Trustee of said Trust. The nature of the suit is a petition to docket said trust and to have the court interpret certain dispositive provisions of said trust.

This summons by publication is specifically directed to the Children of Hurless L. Schwartzkopf and/or the heirs at law of Frank J. Schwartzkopf and Dorothy J. Kemp whose identities and exact address are unknown.

This matter has been set for hearing before the above named court on the 7th day of May, 2018 at 8:30 A.M. If you do not appear at that date and time, your rights may be adversely affected and judgment may be entered against you.
